Print the form below and include it in your shipment so we can handle your request promptly.

There are two options: fill out the form digitally with an online pdf editor or Adobe Acrobat and print to include with your knife shipment. Or, print it first and fill it out by hand.

Pack your knives or other items securely in a box.

IMPORTANT: pack your knives in their original leather sheath or a sheath created from cardboard. For the safety of our postal workers, don’t use a padded or cardstock envelope, please use a cardboard box.

Ship your knives to our factory in Idaho.

We do not supply return labels. Please ship via the carrier of your choice and hold on to your shipment tracking number for reference. See our shipping policies here.
